Habilitation Manager Pee Dee Division- Cheraw Activity Center
Cheraw, SCCHESCO Services is accepting applications and/or letters requesting a transfer for the following position:
Habilitation Manager (Full-time first shift), CHESCO Cheraw Activity Center, Cheraw, SC - Monday through Friday.
Responsibilities include working with direct care staff and consumers to develop a plan of service, based on identified needs determined by an assessment. Monitors consumer progress of goals through ongoing contacts with staff and consumers, and initiates plan amendments and addendums when necessary. Ensures that all other documentation involving the consumers served by the Day Program is accurate. Attends all mandatory meetings. Provides the fiscal office with consumer/worker timesheets and other essential documents. Substitutes for Direct Support Professionals as needed, and works with the Program Coordinator, sharing responsibility for site operations in absence of the Program Coordinator.

PREFERRED QUALIFICATIONS:
- Associate degree in education, psychology, or related human services field.
- One year experience in working with mentally or physically disabled persons is preferred.
- Supervisory experience is a plus.
